Further to our previous updates regarding this project, we are pleased to confirm that we have decided to proceed with the idea of our three Practices (Camrose, Gillies and Hackwood) working as a ‘Primary Care Home’. Primary Care Home is an innovative approach to strengthening and redesigning primary care. Developed by the NAPC, the model brings together a range of health and social care professionals to work together to provide enhanced personalised and preventative care for their local community. Staff come together as a complete care community drawn from; GP surgeries, community, mental health and acute trusts, social care and the voluntary sector, to focus on local population needs and provide care closer to patients’ homes. Primary care home shares some of the features of the multispecialty community provider (MCP) – its focus is on a smaller population enabling primary care transformation to happen at a fast pace, either on its own or as a foundation for larger models.
The recommended size of Practices working together as a Primary Care Home is between 30,000-50,000 registered patients. Combined, we sit at around 45,000 patients which fits in well this is model. Having an integrated workforce to include primary and secondary care as well as social care is a key characteristic of this approach, as is having a strong focus on personalisation of care to create improvements in health outcomes for our patient population. The sharing of risks and rewards by working as a trio is another strong driver for the Primary Care Home initiative.
We recognise that we need to be working differently in order to keep up with the demand that is modern day General Practice. We feel that our three Practices are similar in terms of our patient demographics and also our GPs are similarly minded which sits in very well with the Primary Care Home model.
To start with, we are going to be working together on a small scale, most likely working together to provide clinical services. However, in time, we may expand to look into sharing other non-clinical General Practice workload. We have discussed that this may pave the way towards a merger in the future, if we decide to go down that route. Currently, there are no concrete plans for a merger, but it has certainly not been ruled out.
We will be looking for feedback and suggestions as we all go through this journey together and we will be providing patients with more information as we progress. If anyone has any concerns at this stage, please raise them with the Practice Manager of your Practice. Please do bear in mind that we are only in the very early stages of this project so we might not have all of the answers just yet.
